Radionuclide synovectomy (RSO)

Gentle treatment for painful and swollen joints

What is radionuclide synovectomy?

Radionuclide synovectomy (RSO) is a modern method of treating chronic inflammation of the joint lining (synovitis).
A small amount of radioactive substance – yttrium-90 – is applied to the joint, acting directly on the diseased tissue. The inflamed lining gradually shrinks, leading to relief from pain and swelling.

The advantage is that the method is minimally invasive, does not require surgery, and the effect lasts for months to years.

When is the examination performed?

  • In cases of rheumatoid arthritis of the knee that does not respond to conventional treatment.
  • For psoriatic arthritis with persistent inflammation.
  • For haemophilic arthropathy (repeated bleeding into the joints).
  • In degenerative osteoarthritis with reactive inflammation.
  • In cases of recurrent effusions and joint pain, even if treatment has not yet been effective.

How is the treatment performed?

  1. The procedure is performed in a specialised facility under aseptic conditions.
  2. First, excess effusion is drained from the joint.
  3. A radioactive substance and a small dose of corticosteroid are injected into the joint.
  4. This is followed by a control scan to verify the correct placement of the drug.

The application itself takes only a few minutes; most of the time is spent on preparation and imaging.

What can you expect?

  • The effect sets in within a few weeks to months.
  • The result is pain relief, less effusion and improved mobility.
  • The effect usually lasts for 1–5 years, sometimes longer.
  • The method often delays or completely replaces the need for surgery.

How to prepare

  • Before the procedure, it is necessary to rule out infection (blood tests, possibly puncture).
  • The doctor will explain the details and ask you to sign a consent form.
  • On the day of the procedure, come in comfortable clothing; no other special preparation is necessary.

Is the treatment safe?

Yes. The method is proven and has a high success rate (up to 80% of patients report improvement). Only temporary worsening of symptoms
in the first few days is possible, and very rarely infection or skin reactions.
