What is renal scintigraphy with captopril test?

This examination helps to determine whether your high blood pressure (hypertension) is caused by narrowing of the blood vessels that supply blood to the kidneys. If so, this type of hypertension may be easily treatable.

Why is the examination performed?

  • For long-term high blood pressure that is difficult to lower.
  • In cases of sudden deterioration of previously stable blood pressure.
  • If narrowing of the renal artery is suspected based on ultrasound, CT or magnetic resonance imaging.
  • Before planned procedures (e.g. angioplasty or stenting).

How is the examination performed?

  1. A small amount of radioactive substance is injected into a vein, which allows the function of the kidneys and urine flow to be monitored.
  2. A special camera records the activity of both kidneys.
  3. You will then be given the drug captopril (an ACE inhibitor), which affects the activity of the renal vessels.
  4. This is followed by a second series of images to show whether the narrowed vessel is causing the problem.

The entire examination takes approximately 60–90 minutes and is painless.

How to prepare

  • Some blood pressure medications (especially ACE inhibitors and sartans) must be temporarily discontinued – always follow your doctor's instructions.
  • Drink plenty of fluids before the examination (about 0.5–1 litre).
  • You will need to empty your bladder before entering the examination room.

Is the examination safe?

Yes. The radiation exposure is low and the method is commonly used. The only restriction is pregnancy; in such cases, other procedures are chosen.
